The healthcare provider hereby agrees that the respective doctor of the healthcare provider shall be granted access to the findings and imaging documentation of this doctor’s patients from examinations performed on the basis of an eReferral issued by the same doctor, including their reports (hereinafter referred to as the “Examination Results”), exclusively through the ERADIOLOGIA system, via the account of the respective doctor.

For the avoidance of any doubt, the healthcare provider expressly states that it does not require the preparation or delivery of a printed version of the Examination Results.

The obligation to deliver examination results in electronic form through and to the extent of the electronic record of examination results of joint diagnostic and therapeutic units within the patient’s Electronic Health Book (so-called eHealth system) remains unaffected by this consent.

The person granting this consent hereby expressly declares that they are authorized to grant such consent on behalf of and for the account of the healthcare provider, meaning they are either the healthcare provider or a doctor authorized to perform this act in the name of the healthcare provider.
